Specific Services Provided:

• Facilitate inpatient, outpatient, and home services for injured patients

• Conduct medical evaluations

•  Recommend environmental modifications as needed

• Assist in securing funding for medical equipment, supplies, medications and services

• Provide information to the patient to help facilitate timely and appropriate treatment

• Act as a liaison with patients, medical providers and insurers

• Provide information regarding community programs and services

• Assist with safe and expedient return to work
   

What is a Medical Case Manager?

Medical case managers are registered nurses who have been specially trained to assess and implement care needs and services for medically compromised individuals.
